Please ensure that you read it carefully and complete the details honestly.

If you answer 'YES' to any question, then this will require a signed Fit to Dive Statement from a doctor. This is not asking the Doctor to perform a medical but is rather asking the Doctor to agree the individual's general fitness.

Failure to comply with this requirement could result in you being unable to participate in the booked course and the loss of any fees paid.
